Important Medicare Updates for Anesthesia and Sedation Coverage in Facet Joint Injections

NGS (L35936) – Effective 7/31/2024

  • Moderate or deep sedation, general anesthesia, and monitored anesthesia care (MAC) are not considered medically reasonable and necessary during facet procedures such as intra-articular (IA) injections, medial branch blocks (MBB), and facet cyst aspiration/rupture.
  • The use of moderate anesthesia for radiofrequency ablation (RFA) will be considered on an individual basis with documented medical necessity (e.g., longstanding history of inability to cooperate, prohibitive medical conditions, inability to remain motionless). Patient anxiety or preference alone is insufficient justification.

CGS (L38773) – Effective 7/7/2024

  • Similar limitations as NGS regarding moderate or deep sedation, general anesthesia, and MAC for facet injections.
  • Moderate sedation for RFA or cyst rupture/aspiration will be considered on a case-by-case basis with appropriate medical documentation.

Novitas (L34892) & First Coast (L33930) – Effective 8/11/2024

  • Consistent with the aforementioned policies, moderate or deep sedation, general anesthesia, and MAC are generally not covered for facet injections.
  • Individual consideration for moderate sedation in specific cases of RFA or cyst rupture/aspiration with necessary medical documentation.

Noridian (L38803) – Effective 7/15/2024

  • Adopts similar policies, emphasizing the necessity of medical documentation for moderate sedation in special cases.

Palmetto (L38765) – Effective 7/7/2024

  • Reiterates the limitations on anesthesia and sedation, aligning with other contractors in requiring specific medical justification for moderate sedation use in RFA or cyst procedures.
